Teach-back in Indian Country helps clinicians confirm that patients understand their care plan by inviting them to explain the next steps in their own words, making clear communication a practical skill to teach every incoming healthcare team member.
Teach-Back Checks the Explanation
The Indian Health Service’s teach-back guidance describes a method for confirming that information was explained in a way the patient understands. If the explanation does not come through clearly, clarify it and invite the patient to explain again. Written materials can reinforce the conversation, but repeating a handout word for word does not establish understanding.
For teams serving distinct Tribal nations, avoid assumptions about a person’s knowledge based on age, identity, or education. Focus on the explanation and the individual patient’s questions, preferences, and next steps.
Practice Before the Next Patient Visit
The Agency for Healthcare Research and Quality’s practical toolkit recommends giving information in small portions and checking understanding throughout the encounter. It also recommends training clinical and nonclinical staff. A team can rehearse a medication explanation, an appointment instruction, or a home care conversation before using the approach with patients.
Ask a colleague to explain the plan back, then revise unclear wording. For a skill such as inhaler use, ask for a demonstration. Invite questions with a respectful tone and explain that the purpose is to check your communication.
Build Clear Communication Into Orientation
Make teach-back part of orientation for permanent and temporary clinicians, with local staff helping select realistic scenarios. Ask team members to practice explaining one important instruction without medical jargon. Recognize that cultural awareness means listening to each patient rather than applying a script to every Native community. Our perspective is that readiness includes explaining care clearly. Start with one shared practice session and keep improving how your team communicates with Our Relatives.
