Syphilis testing in Indian Country now depends on the whole care team, because IHS recommends annual testing for everyone 13 and older, three tests during pregnancy, and field treatment for patients and their partners.
What the 2026 IHS Syphilis Guidance Asks of Clinics
In an August 19, 2026 IHS letter to Tribal and Urban Indian leaders, Chief Medical Officer Loretta Christensen, M.D., confirmed that earlier syphilis recommendations remain in effect with added considerations.
The enclosed IHS syphilis testing and treatment guidelines call for testing at the first prenatal visit, the start of the third trimester, and delivery. They also call for universal screening and treatment in urgent care and emergency departments, Express STI testing at all sites, and penicillin G benzathine for contacts and partners, including non-beneficiaries.
Why Syphilis Testing in Indian Country Is a Workforce Issue
The need is urgent. IHS syphilis statistics drawn from CDC data show a congenital syphilis rate of 680.8 per 100,000 live births among American Indian and Alaska Native infants in 2023, when Native babies were 0.7% of births but 4.6% of cases. A January 2026 NCHS report on maternal syphilis by Elizabeth Gregory and Danielle Ely found the rate among American Indian and Alaska Native mothers rose 52% from 2022 to 2024.
Each missed test is a missed chance to protect a family, and every step needs a trained person.
How Tribal Health Leaders Can Staff the Response
Leaders should assign each step to a named role, from prenatal and emergency nurses to public health nurses providing field treatment, pharmacists watching penicillin supply, and community health representatives who know where Our Relatives live. Cover nights, weekends, and turnover so results never sit unread.
