Medicaid work requirements take effect in most states by January 1, 2027, and because American Indians and Alaska Natives are exempt, trained Tribal benefits staff are now a key protection against eligible Relatives losing coverage by mistake.

What the Medicaid Work Requirements Rule Says

CMS issued its Medicaid community engagement interim final rule on June 1, 2026. Many adults covered through Medicaid expansion must show 80 hours a month of work, school, or community service, checked at application and renewal. People the state cannot verify get 30 days to respond before denial or disenrollment. American Indians and Alaska Natives are listed among the exempt groups.

In a September 2026 post by Alex Sampson, the National Council of Urban Indian Health (NCUIH) urged states to accept self-attestation and avoid reverifying Native status.

Why Exempt Patients Can Still Lose Coverage

An exemption protects a patient once the state has it on record. Missed mail, outdated addresses, and unanswered notices can still end coverage through procedural disenrollment, the risk NCUIH flagged most directly. That loss reaches clinics too.

The National Indian Health Board’s April 2026 budget analysis reports that Indian health providers receive 30 to 60 percent of their funding from Medicaid billing. Writing for Great Plains Tribal Health, Tosa Two Heart advises Tribal citizens to open all Medicaid mail, keep contact information current, and call their Tribal health program with questions.

How Tribal Health Leaders Can Prepare Now

Leaders should map who handles Medicaid notices, train registration staff and patient benefits coordinators on the exemption, and set aside staff time for renewal questions as states launch their systems. Ellsworth, a Native-owned healthcare staffing company serving IHS, Tribal and 638, and rural facilities, views front desk and benefits roles as part of the care team, because a lapsed enrollment often becomes a missed visit.

If your facility needs added registration or benefits support before January, reach out to Ellsworth so Our Relatives keep the care they rely on.
